GUIDE

EMsculpt NEO in London: how it works, the body areas, the sessions and what it costs

Book a free consultation

Consultations and follow-ups are free

This guide is for someone deciding whether to book, not for someone who has already decided. It goes area by area, then session by session, and it gives the manufacturer's figures and the independent reviews' figures side by side, each labelled. Where we describe our own protocol we say so. Everything here was reviewed by Dr Marie Duggan, GMC 7488383, and your suitability is only ever settled at a free consultation at 8 Devonshire Place.

Two energies, one applicator: what happens in the 30 minutes

A flat applicator is strapped over the muscle group you want treated. For 30 minutes it does two things at once.

The first is an electromagnetic field, which the manufacturer (BTL Industries) calls HIFEM. It makes the muscle under the applicator contract hard and involuntarily, in waves; the manufacturer's advisors say contractions this strong cannot be reached voluntarily. A 2019 paper on the original device counted almost 20,000 pulses from the applicator in half an hour: strong, involuntary contractions in waves for the whole session.

The second is radiofrequency, which warms the muscle and the fat over it. On the manufacturer's account the warmth prepares the muscle before the contractions build, like a warm-up. Small human studies measured the fat layer at 42 to 45 degrees. In that range fat cells are reported to start programmed cell death, apoptosis, and the body is thought to clear them over the weeks that follow. The original EMsculpt had the electromagnetic part only and no fat-reduction clearance; the radiofrequency is what NEO added.

Nothing is cut or injected and nothing goes on the skin: no gel, no numbing. You lie still, the intensity is raised in steps to a level you can hold, and you talk to the clinician throughout. Two applicators can run at the same time, so a pair of areas such as both flanks is treated in the same 30 minutes.

Area by area: what is cleared, what was measured, what to expect

EMsculpt NEO is FDA-cleared in the United States, which is not the same as approved. Clearance means the regulator accepted the device as substantially equivalent to one already on the market, for a stated use. The uses are specific, and they differ by area. The table gives, for each area, the clearance, the figures that exist, and what that means in the mirror.

AreaUS clearanceWhat was measuredWhat that means for you
AbdomenFat and circumference reduction (2019); muscle toning (2018 to 2019)Sham-controlled trial, 72 people: fat 28.3% thinner (about 7.6 mm), rectus muscle 24.2% thicker (about 2.3 mm) at three months, held at six. Manufacturer's single-arm study, 42 completed: fat down 29.8%, abdominal circumference down 3.2 cm on average, 88% lost at least 1.5 cmA flatter, firmer abdomen where the layer is soft and the muscle underneath is slack. A firm, pinchable bulge is a fat-freezing question, which we also treat
FlanksFat and circumference reduction (2023)MRI study (Cohen 2024): fat 28.8% thinner, oblique muscles 27.2% thicker after four weekly sessions. Manufacturer's ultrasound study: 5 mm average reductionThe soft part of a "love handle". Both sides in one session with two applicators
Upper armsFat reduction (2022); muscle toning (2018 to 2019)Jacob and Weiss 2023: fat 25.5% thinner, triceps 23.9% thicker, 25 people scanned at three monthsTriceps definition and a thinner layer at the back of the arm. The US label limits arms to a BMI of 30 and to skin types II and III
ThighsFat reduction (2021); muscle toningCleared for fat reduction on the manufacturer's data; we quote no thigh figureFat on the thighs and tone of the thigh muscles, judged at your three-month review in dated photographs
ButtocksMuscle toning only; no fat indicationMRI study recruiting 67 (54 analysed): gluteal muscle about 25% thicker at three monthsLift and shape from a thicker muscle rather than from fat loss; for a fat concern the clinician says at the consultation what suits it
CalvesMuscle toning only; no fat indicationCleared for muscle toning; we quote no calf figureTone, not fat reduction

Skin types: the fat clearances covered skin types I to III first; from 2021 all skin types for the abdomen and thighs; flanks I to IV; arms II and III.

Every percentage in that table comes from a manufacturer-linked study, and the independent reviews below put the same results in millimetres. We quote the figures because they are the best that exist; what you can expect is judged at the consultation, on your own layer and muscle.

Original EMsculpt or EMsculpt NEO: what changed

This clinic treated with the original EMsculpt before NEO existed, and figures for the two machines are often quoted side by side. They are different machines and the difference matters when you compare quotes.

The original EMsculpt runs the electromagnetic field alone. In the manufacturer's studies it reduced fat by about 19% and thickened muscle by 15% to 19%. It has no fat-reduction clearance in the United States.

EMsculpt NEO adds the radiofrequency. The manufacturer's own comparison puts the fat change at about 19% for the original against about 30% for NEO, and the muscle change at about 16% against about 25%. The buttock MRI study in the table found the same pattern: muscle about 25% thicker with NEO against about 16% with the original. A 21-patient uncontrolled study of the muscle-only device, by manufacturer advisors, scanned people about a year after a course and found the fat still about 15% thinner and the muscle about 19% thicker than at the start.

We treat with EMsculpt NEO. If a quote elsewhere looks low, the first question is which machine it is for.

A course, session by session

The trials that produced the figures above ran sessions a week apart: three sessions in the sham-controlled trial and in the manufacturer's abdomen study, four in the flank, arm and buttock studies. Our course is four sessions, five to seven days apart, and we review you at three months. This is our protocol, built on those trials.

WhenWhat happens
Consultation (free)A clinician in our doctor-led team pinches the fat layer, checks the muscle beneath it, asks about implants, metal, surgery and pregnancy, and writes the plan: which area, how many sessions, and the price
Session 1The applicator is strapped on, the intensity rises in steps and you feel the muscle contract in waves. The area is pink and warm for an hour or so afterwards. The muscle aches like a hard training session for a day or two
Sessions 2 and 3The intensity is higher as you get used to it. In the manufacturer's FDA study 93% of people reported none to minimal discomfort
Session 4The last session of the course. In our experience many people have felt the muscle working since session 1, and the firmer look arrives two to four weeks after this one
Three monthsThe free review. The trials measured their main result at three months, and this is when we compare your dated photographs with the first set
AfterwardsIn our experience a single session every three to six months keeps the tone

Why four sessions and not three? Three sessions is what the sham-controlled trial used; in our experience the fourth is where the firmness becomes visible rather than felt. A single session shows you what the treatment feels like; it is not a course.

If you miss a week, tell us and we fit the next session in. The trials ran sessions weekly, so we keep to that rhythm where we can, and one missed week is not a reason to restart a course.

Who gets the most from it, and who needs a different plan

Three people who do well:

  • Someone who trains and eats carefully and still has a soft lower abdomen or soft flanks that will not shift. The layer is thin enough to pinch, the muscle underneath is there, and the treatment firms both.
  • Someone whose abdominal wall has been weak since pregnancy and who has fully recovered from the birth. The muscle thickening is the part that helps here: we treat for a firmer abdominal wall rather than a closed gap, and the clinician examines the abdominal wall at the consultation first.
  • Someone who wants more definition on the arms, the flanks or the buttocks: a firmer muscle, a thinner layer on the arms and flanks, and a change that shows in how clothes fit.

Three people it will not do enough for, and we say so at the consultation:

  • Anyone whose aim is weight loss. Body weight did not change in the trials. It reshapes one muscle group at a time, alongside diet and exercise rather than instead of them.
  • Anyone whose concern is loose skin or cellulite. Those are skin problems with their own treatments, and we provide them: dimpled skin is an EMtone question; loose skin is a skin-tightening question. We will tell you at the consultation which fits, and some people combine one with EMsculpt NEO.
  • Anyone with a firm, pinchable bulge. A firm bulge is a fat-freezing question; we provide CoolSculpting as well and will say which suits you, or whether the two in sequence make sense.

On body size: in our experience the best results come at a BMI under about 30. The manufacturer accepts up to 35, which was the entry limit of its trial, and the sham-controlled trial found no link between BMI and the result. The US label sets a BMI limit of 30 for the upper arms only. Men and women took part in the sham-controlled trial, which found no link between sex and result.

Before you book: the checks we make

This is the list clinics relay from the manufacturer's guidance; the full list is in the manufacturer's manual, and we check every one of these against it at the consultation. Tell us when you book if any applies.

  • An implanted electronic device: pacemaker, defibrillator, neurostimulator, drug pump.
  • Metal in or near the treated area: a copper coil, plates, screws, a joint replacement. Most hormonal coils contain little or no metal (some carry a small silver ring); bring the name of yours and we check it.
  • Pregnancy, and breastfeeding on some clinic sheets. After a birth we wait until you have fully recovered.
  • A hernia in the area.
  • Cancer in the area, a fever, epilepsy, a heart condition, a bleeding disorder or blood-thinning medication.
  • Recent surgery, an open wound or an infection in the area, including a caesarean scar that has not healed. The manufacturer's FDA filing also lists injured or impaired muscles.
  • A recent injectable treatment in the same area: tell us when and what, and we time the sessions around it.

If you have had abdominal surgery or a diagnosed diastasis, say so. The clinician examines the abdominal wall at the consultation and plans around it, and sometimes advises against treatment.

Reading the numbers: percentages, millimetres and the mirror

The manufacturer's headline is "30% fat, 25% muscle". Those are averages from the manufacturer's studies, most of them in the table above, measured as thickness on an ultrasound or MRI scan. What they mean for you is easier to picture in millimetres and centimetres.

Independent reviews that pooled the published studies (Swanson 2023, Kohan 2024, Swanson 2025) put the average change at about 5.5 to 8 mm less fat and 2 to 5.5 mm more muscle, depending on the area and the device generation. That is millimetres and centimetres: in the manufacturer's abdomen study the waist measured 3.2 cm smaller on average and 88% of people lost at least 1.5 cm; your own dated photographs at three months are the measure that matters.

A thinner layer and a firmer muscle are visible on the right person and invisible on the wrong one, which is why the consultation starts with a pinch test: it tells us quickly whether EMsculpt NEO is likely to show on you. Measure your result in how clothes fit and in dated photographs under the same light, not on the scales.

This guide shows no manufacturer model photographs: the Advertising Standards Authority for Ireland ruled in that one such image, a very defined torso, could give the impression that any user could achieve it. At the consultation we show you dated photographs of our own patients where we have them, with their consent.

Side effects, and what to tell us

The treated area is pink and warm for about an hour. The muscle is sore for 24 to 48 hours, in the way it is after a hard session in the gym.

No serious adverse event has been reported in the published studies. Some consent forms in the United States list localised overheating, a heavier period in the days after treatment, and inflammation of the fat layer (panniculitis); we found no published case report of the last.

Tell us if the area is painful rather than sore, if the skin blisters, or if it changes colour. Those are not expected and we want to see them.

Keeping the result

The fat change holds while your weight holds. The muscle change behaves like training: it fades if you stop. The people who keep their result keep training.

Keep your weight stable through the three months after the course, because that is when the result is measured and because a gain in that window hides it. Come to the free review at three months. After that, a single session every three to six months keeps the tone in our experience.

What it costs at our Harley Street clinic

EMsculpt NEO is from £100 per session, and we confirm your price in writing at the free consultation before you commit to anything. We price match any comparable written quote. Finance plans are available, subject to status, with terms that depend on the amount and are set out in writing. Consultations and follow-up reviews are free whatever you decide.

Your written plan names the machine, the area, the number of sessions and the price, so you can compare any other quote on the same terms.

Preparing for a session, and after it

Before: eat and drink normally, wear loose clothes you can pull aside, and take off jewellery and anything metal near the area. No numbing is used and no gel, so there is nothing to apply at home. If you are unsure about a coil, an implant, a scar or a recent treatment, tell us when you book rather than on the day.

After: drink normally, train as you usually do, take an ordinary painkiller if the muscle aches, and keep your weight stable through the three months. Book the free review at three months and bring, or let us take, dated photographs under the same light as the first set.

Frequently asked questions

Will I lose weight with EMsculpt NEO?

No: it changes shape, not weight. Body weight did not change in the trials; the treatment thins the fat layer over one muscle group by millimetres and thickens the muscle underneath, and in the manufacturer's abdomen study the waist measured 3.2 cm smaller on average. It sits alongside diet and exercise, not instead of them, and the consultation tells you whether it is likely to show on you.

How does EMsculpt NEO compare with EMtone?

They do different jobs. EMsculpt NEO works on the muscle and the fat layer over it. EMtone, which we also provide, combines radiofrequency heat with targeted pressure energy to reduce the appearance of cellulite. A soft layer over a slack muscle is an EMsculpt NEO question; dimpled skin is an EMtone question; loose skin is a skin-tightening question, which we also treat. Some people have both, one after the other, and we will say which order makes sense for you.

Can two areas be treated in one visit?

A pair of areas such as both flanks runs in the same 30 minutes with two applicators. Two different muscle groups, such as the abdomen and the buttocks, are separate 30-minute sessions. Each area has its own course of four.

Is EMsculpt NEO cleared for the thighs and calves?

The thighs are cleared in the United States for fat reduction (2021) and for muscle toning. The calves are cleared for muscle toning only, with no fat indication. We put no figure on either area; we judge your result at the three-month review in dated photographs.

How soon after a birth or a caesarean can I start?

Our rule: fully recovered from the birth and any caesarean scar fully healed. Recent surgery and an unhealed wound in the area rule the treatment out on their own. At the consultation we examine the abdominal wall and ask about any diagnosed diastasis before we treat; the aim is a firmer, thicker abdominal muscle, which is what the studies measured.

How soon before a holiday or an event should I start?

Count back about four months. The course takes about three weeks from the first session to the last, the area starts to look firmer two to four weeks after the last session in our experience, and the trials measured their main result three months after the last session. A course started a fortnight before a holiday will be felt but not yet seen.

What if I miss a week between sessions?

Tell us and we fit the next session in. The trials ran sessions weekly, so we keep to that rhythm where we can. One missed week is not a reason to start the course again.

Can I train on the day of a session?

Yes. Nothing about the treatment stops you training before or after, and the people who keep their result are the ones who keep training. If the muscle is sore the next day, treat it as you would after a hard session and train lighter.

What should I wear and bring?

Loose clothing you can pull aside from the area, nothing metal near it, and the name of your coil or implant if you have one. Nothing is applied to the skin and there is no numbing, so there is nothing to bring or do at home.

Is EMsculpt NEO CQC regulated?

Our clinic is CQC registered. Treatments carried out purely for cosmetic purposes, such as EMsculpt NEO for body shape, are outside the activities the Care Quality Commission regulates, so here is what we give you for this one: a genuine BTL device, a clinician who places the applicator and reads the contraindications, a plan in writing, and a doctor to answer to if something goes wrong. This guide was reviewed by Dr Marie Duggan, GMC 7488383.

How many sessions do the arms need?

Four, five to seven days apart, which is our protocol for every area. The arm study (Jacob and Weiss 2023) used four sessions and scanned people at three months, which is where the 25.5% fat and 23.9% triceps figures come from.
